卡培他滨单药及联合奥沙利铂方案治疗老年晚期胃癌的疗效比较

目的比较卡培他滨单药及联合奥沙利铂方案治疗老年晚期胃癌的疗效及安全性。方法 46例老年晚期胃癌患者随机分为对照组和治疗组,分别应用卡培他滨单药及联合奥沙利铂方案治疗,21d为一个周期。治疗2个周期按RECIST标准评价,并比较两组的近期疗效及毒副反应。结果对照组和治疗组有效率分别为31.82%和50.00%,中位疾病进展时间(TTP)分别为4.2个月和6.1个月,中位生存期(OS))分别为8.7个月和11.9个月。治疗组高于对照组,但两组有效率比较无统计学意义(P>0.05);主要毒副反应为手足综合征、恶心呕吐、腹泻、口腔炎症、白细胞减少及神经毒性等,一般为Ⅰ～Ⅱ度,均可耐受。结论两种方案治疗老年晚期胃癌有效率高疗效相近,但联合治疗组有更长生存期趋势,有待于进一步研究。

The comparision of the medical effcts of capecitabine only and capecitabine plus oxaliplatin in the treat- ment of elderly patients with advanced gastric cancer

Objective To evaluate the effects and safety of capecitabine only and capecitabine plus ox- aliplatin in the treatment of elderly patients with advanced gastric cancer. Methods Forty-six elderly patients with advanced gastric cancer were randomly divided into comparative group and treatment group, were treated with capecitabine only and capecitabine plus oxaliplatin. The treament was repeated every 3 weeks. The effect was evaluated according to RECIST after two cycles. Results The total effective power of the two groups was 31.82% and 50. 00%. The median time to progression was 4. 2 months and 6. 1 months. The medical survival period was 8. 7 months and 11.9 months. The effect of the treatment group was better than that of the compara- tive group, but there was no significant difference between the two groups （ P 〉 0. 05 ）. The main toxicitics was hand-foot syndrome, nausea, vomiting, diarrgea, oral cavity ulcer, leueopenia, neurotoxicity, most was grade Ⅰ - Ⅱ and well tolerated. Conclusion Capecitabine only and capeeitabine plus oxaliplatin was effects and safety for the treatment of elderly patients with advanced gastric cancer. But the second has longer surival period and further study should be carried out.
