125Ⅰ粒子植入治疗胰腺癌的疗效观察

目的 评价CT引导下125Ⅰ粒子植入治疗胰腺癌的临床价值,总结此类手术的经验和技巧.方法 回顾性分析2005年11月至2009年5月成都军区总医院普通外科中心21例经CT引导下125Ⅰ粒子植入治疗胰腺癌患者的临床资料.全部患者经CT或MRI检查并行病理穿刺活检后临床诊断为胰腺癌.采用计算机立体定位计划系统(TPS)计算布源,在CT引导下将125Ⅰ粒子植入胰腺癌灶内,术后行CT检查观察患者的临床受益反应情况及患者生存情况.结果 全组患者中位生存期为13.5个月.术后3个月行CT检查,完全缓解2例(9.5%);部分缓解6例(28.6%);稳定12例(57.1%);进展1例(4.8%);总有效率为38.1%.全组患者疼痛缓解率为90.4%,明显减少镇痛药物用量,体力状态评分增高.结论 CT引导下125Ⅰ粒子植入治疗胰腺癌创伤小,恢复快,并发症发生率低,生活质量改善明显,在一定程度上提高了患者生存率,是一种安全有效的姑息治疗胰腺癌方法.

Abstract：

Objective To investigate the clinical value of computed tomography(CT) guided 125Ⅰ implantation in treatment of pancreatic cancer. Methods A retrospective study was employed to analyze the clinical data in 21 pancreatic patients who had received the 125Ⅰ implantation at Chengdu Military General Surgery Center from November 2005 to May 2009. A follow-up of 1-25 months was conducted. Results 125Ⅰ implantation radiotherapy for pancreatic cancer showed a longer survival time. The whole pain relief rate was 90.4%. Meanwhile there were significantly reduced analgesic drug consumption and improved the physical state of Karnofsky performance score (KPS). The efficiency of this therapy was 38.1% judged by CT examination. Conclusion 125Ⅰ implantation is an effective and safe treatment for pancreatic cancer.
