血清糖类抗原及癌胚抗原联合检测在胰腺癌诊断中的意义

目的 评价联合检测血清糖类抗原(CA)19-9、CA125、CA242及癌胚抗原(CEA)4项指标对胰腺癌的诊断价值.方法 选取60例胰腺癌患者作为胰腺癌组,30例同期住院的良性胰腺疾病患者作为良性胰腺疾病组,30例同期体检合格的健康成人作为正常对照组各30例.采用放射免疫分析法检测3组入选对象的CA19-9、CA125、CA242、CEA血清水平并进行对比.结果 胰腺癌患者血清CA19-9、CA125、CA242、CEA值均明显高于正常对照组及良性胰腺疾病组,差异均有统计学意义(均P＜0.05);CA19-9、CA125、CA242、CEA 4项指标联合检测的敏感性(97.6%)和特异性(98.8%)较单测任何一项(CA19-9为81.5%、83.6%,CA125为61.2%、65.4%、CA242为63.6%、74.5%,CEA为45.1%、52.6%)均有提高,差异均有统计学意义(均P＜0.05).结论 联合检测血清CA19-9、CA125、CA242及CEA能提高对胰腺癌的阳性检出率,临床诊断价值更大.

Abstract：

Objective To study the diagnostic value of combined detection of four tumor markers carbohydrate antigen(CA) 19-9,CA125,CA242 and carcinoembryonic antigen(CEA)] in patients with pancreatic cancer.Methods Serum levels of CA19-9,CA125,CA242 and CEA in 60 cases of pancreatic cancer,30 cases of benign pancreatic lesions and 30 normal controls were detected by radioimmunoassay. Results Serum CA19-9,CA125,CA242 and CEA levels in pancreatic cancer patients were significantly higher than those in normal controls and benign pancreatic lesions patients (P ＜0.05). If serum CA19-9,CA125,CA242 and CEA were detected simultaneously,the sensitivity and specificity of diagnosis of pancreatic cancer could be improved. With combined detection of CA19-9,CA125,CA242 and CEA in patients with pancreatic cancer,the sensitivity and specificity was 97.6% and 98.8% respectively. Conclusion Combined determination of serum CA19-9,CA125,CA242 and CEA can improve the sensitivity and specificity in diagonosis and differential diagnosis of pancreatic cancer.
