18F—FDG PET／CT对乳腺癌分期及疗效监测的价值探讨

目的探讨18F—FDG PET／CT在乳腺癌诊断及术后疗效监测的应用价值。方法对2006年1月～2009年12月笔者所在医院收治的的76例病理类型最终确诊为乳腺癌的病例行”F—FDGPET／CT显像和胸部诊断CT扫描，检查范围包括乳房原发肿瘤、淋巴结及全身器官，并将18F—FDG PET／CT和胸部CT检查的结果进行对比。结果18F—FDG PET／CT检查对乳腺癌诊断的灵敏度为91．3％，特异性为53．1％，准确性为85．3％；而胸部诊断cT的灵敏度检查为62．1％，特异性为29．8％，准确性为55．6％。在对淋巴结转移的诊断上，18F—FDG PET／CT的灵敏度（99．8％）高于胸部诊断CT的灵敏度（53．1％）。而在对肺部转移灶检测上胸部CT的灵敏度（989％）高于18F—FDG PET／CT（42．9％）。结论18F—FDG PET／CT在对乳房原发肿瘤、淋巴结转移和术后诊断复发的发现和判断上显示出明显优势；但胸部CT在发现肺部转移灶的作用不容忽视，其地位不可取代。

18F - FDG PET / CT staging of breast cancer and the effect of the value of monitoring

Objective To investigate the applicational value of ISF - FDG PET/CT in the diagnosis of breast cancer and the curative effect. Methods 76 cases who were finally diagnosed of breast cancer were collected the 18F - FDG PET/ CT imaging and chest CT scan. The scope of the examination included breast primary tumors, lymph nodes and systemic organ. The 18F - FDG PET/CT and results of the chest CT examination were compared. Results The sensitivity of 18F - FDG PET/CT examination for breast cancer diagnosis is 91.3%, the specificity rate is 53.1%, the accuracy rate is 85.3% And the sensitivity of chest diagnosis CT examination were calculated to be 62. 1% , the specificity rate occupied 29.8% , accuracy rate is 55.6%. In the diagnosis of lymph node metastasis, 18 F - FDG PET/CT sensitivity （99.8 per cent） is better than the sensitivity of chest CT （53. 1% diagnosis）. While in lung metastases detection on chest CT the sensitivity （98.9%） is better than 18F - FDG PET/CT （9.5%）. Conclusion 18F - FDG PET/CT showed obvious advantages In the diagnosis of breast primary tumors,lymph node metastasis and discovery of recurrence;But the status of chest CT in the discovery of lung metastases should not be ignored.
