手用ProTaper根管扩大系统联合标准不锈钢H锉在磨牙根管治疗中的应用

摘    要:【目的】评估手用ProTaper根管扩大系统联合标准不锈钢H锉在磨牙根管治疗中的应用效果。【方法】选择门诊患者的162例患牙随机分成3组。A组用手用ProTaper根扩系统与标准不锈钢H锉进行联合根管预备;B组用手用ProTaper根扩系统进行根管预备;C组用标准不锈钢K锉进行根管预备,3组均以常规侧方加压法进行根管充填。摄片比较3组根管充填质量、术后反应、根扩操作时间及断针情况。【结果】A组根扩操作时间最短,其次是B组,C组所用时间最长,差异有显著性(P〈0.01);3组含断针根管比率为A组〈C组〈B组,差异有显著性(P〈0.05);术后急性反应A组与B组差异无显著性(P〉0.05),与C组有显著差异性(P〈0.01);根管充填后摄片结果比较A组与B组差异无显著性(P〉0.05),与C组有显著差异性(P〈0.01)。【结论】采用手用ProTaper扩大针与标准不锈钢H锉联合扩大根管能有效减少断针的发生,同时能在一定程度上缩短治疗时间,提高根充质量。

Abstract:Objective] To assess the application of hand-used ProTaper instruments in combination with standard stainless steel H-file in endodontic treatment of molar canals. Methods] A total of 162 cases were chosen randomly and divided into 3 groups. Group A was prepared by hand-used ProTaper instruments and standard stainless steel H-file with combined technique; Group B was prepared by hand-used ProTaper instruments; Group C was prepared by standard stainless steel K-file, All three groups were obturated with the cold lateral condensation technique. The quality of obturation was determined by radiograph, and the ratio of post-operation emergencies and working time were measured and analyzed. Incidence of instrument fracture was investigated. Results] A group of operating time was the shortest, followed by Group B and group C was the longest (P〈0.01). Comparing the ratio of root canal contained instrument fracture with the three groups was A 〈 C 〈 B ( P 〈 0.05). There was no statistical difference between A group and B group in the rate of post-operation emergencies (P 〉 0.05). There was statistical difference between A group and C group in the rate of post-operation emergencies (P 〈 0.01). There was no statistical difference between A group and B group in the rate of best root canal obturation (P 〉 0.05). There was statistical difference between A group and C group in the rate of best root canal obturation (P 〈 0.01). Conclusions] The appliance of hand-used ProTaper instruments in combination with standard stainless steel H-file to root canal preparation may improve the quality of obturation and shorten the working time and reduce instrument fracture.
