DRUG RESISTANT PULMONARY CHORIOCARCINOMAMETASTASIS TREATED BY LOBECTOMYREPORT OF 29 CASES
Abstract:From 1965 t0 1976, lobectomy was performedin 29 cases of choriocarcinoma with pulmonarymetastasis resistant to chemotherapy. Followingsurgery l'7 survived, 9 0f whom have been fol-lowed up for more than 5 years, they are ingood health The factors favorably influencingprognosis are: a. solitary lesion on chest film,b. negative urinary HCG determination by toadconcentration test or hemoagglutination inhibi-tion test after chemotherapy, c. improved surgi-cal procedure-ligation of the pulmonary veinsfirst to prevent dissemination during surgicalmanipulation, d. presence of fibrotic tissue cap-sule around the lesion on pathologic examinationof the resected lung specimens and e. no eleva-tion or only temporary elevation of urinary HCGlevel after surgery. Metastasis to other organs did not contrain-dicate lobectomy provided the metastatic focihad been already controlled by chemotherapy.In our series, among the survivors there were3 with metastasis to the brain, 1 to the bladderand l to the rectum.
