肝脏炎性假瘤的CT诊断

目的：探讨肝脏炎性假瘤（ＩＰＬ）的ＣＴ表现，材料与方法，收集１６例ＩＰＬ的临床及ＣＴ资料，ＣＴ所见与手术及病理对照，其中８例为常规扫描后行病灶团注法动态ＣＴ扫描，结果：ＩＰＬ的ＣＴ表现可分５类，（１）病灶边缘清楚，增强无明显强化，其病理主要为大片凝固坏死及周边纤维组织包绕；（２）病灶边界清楚，增强后轻至中度强化，其病理主要为大片凝固坏死及较多的炎细胞浸润；（３）病灶平扫呈低密度，增强后经化为等密度

CT Diagnosis of Inflammatory Pseudotumor of the Liver

Objective: To investigate the CT features of inflammatory pseudotumor of the liver (IPL).Materials and Methods:The CT images and clinical data of 16 cases of IPL were reviewed. All patients underwent routine CT examination and 8 of them underwent dynamic CT scan. Their CT manifestations were compared with pathology.Results:The varied CT manifestations of IPL could be divided into five types: (1) The margin of IPL was clear and there was no enhancement after contrast medium had been injected. The pathology revealed large areas of solidified necrosis surrounded by large amount of fibrous tissue; (2) The magin of IPL was clear and there was slight to moderate enhancement after the injection of contrast medium. The pathology revealed large areas of solidified necrosis and inflammatory cells infiltration in the lesions; (3) The low density lesion became homogeneous after the injection of contrast medium. The pathology revealed a lot of inflammatory cells and fibrous tissue in the lesion; (4) The IPL lesions with unclear margin before and after the injection of contrast medium. The pathology revealed that the granulation tissue was mixed with the residual liver tissue; (5)The shapes of some IPL were special. They looked like dumbbell or consisted of small merged ones and there was also no enhancement of the lesions after the injection of contrast medium. Conclusion:Dynamic CT was recommended when the diagnosis of IPL was considered by routine CT and it was much valuable.
