改良腮腺良性肿瘤切除术68例临床研究

目的改良腮腺良性肿瘤切除术,保留耳大神经、腮腺嚼肌筋膜、腮腺主导管及部分腮腺组织,以减少腮腺良性肿瘤术后并发症。方法对68例腮腺良性肿瘤,根据肿瘤所在的部位,分别采取解剖面神经总干、颞面干分支或颈面干分支做腮腺肿瘤的区域性切除术。结果 68例手术均顺利完成,术后无涎瘘发生,保留耳大神经,耳垂及耳周麻木在6月～9月恢复,暂时性面瘫5例,术后1周～6月恢复。2例出现Frey’s综合征。术区凹陷不明显。结论改良术式切除腮腺良性肿瘤较传统术式降低了并发症。

Clinical study of the improved benign parotid tumor resection in 68 cases

Objective: To reduce the postoperative complications of benign parotid tumor by improving the tumor resection,including retaining great auricular nerve,parotideomasseterica fascia,parotid major duct and parts of the parotid gland.Methods: Sixty-eight cases of benign parotid tumor were included in the study.According to the site of tumor,anatomies of the general facial nerve,temporofacial trunk branch and cervicofacial trunk branch were conducted,or the regional resection of parotid gland tumors were carried out.Results: Sixty-eight operations were completed successfully without any postoperative salivary fistula.The great auricular nerve was reserved and the earlobe and periotic numbness recovered in 6~9 months.Five cases of temporary facial paralysis recovered in a week to 6 months after the operation.Frey’s syndrome occurred in two cases.Hollow in operative region was not obvious.Conclusion: Improved surgical removal of benign parotid tumor described above lowers the postoperative complications,as compared with the routine surgical procedures.
