腹膜后腹腔镜下Anderson-Hynes手术

摘    要:目的 介绍腹膜后腹腔镜肾盂成形术。方法 采用后腹腔途径对5例确诊为肾盂输尿管连接部(UPJ)梗阻的患儿施行腹腔镜离断性肾盂成形术。结果5例手术全部成功,手术时间130~190min。术中出血20~50ml,术后3~24个月B超复查,手术侧肾积水基本消失,3例术后1年IVU检查UPJ吻合口无狭窄,肾盂输尿管排尿功能好。结论后腹腔镜离断性肾盂成形术是治疗UPJ梗阻有效、安全及微创的方法。

Abstract:Objective To evaluate the technique of laparoscopic pyeloplasty for ureteropelvic junction obstruction (UPJO).Methods Five cases with UPJO underwent retroperitoneal laparoscopic dismembered Anderson-Hynes pyeloplasty in this institute. The details of procedure were described. Follow up data of B ultrasound and intravenous urography (IVU) investigations were recorded and analyzed. Results All operations were performed under retroperitoneal laparoscopy. The operation time ranged from130 to 190 min and the blood loss was about 20 to 50 ml. The hydronephrosis disappeared within 3 to 24 months after operation as shown by B Ultrasound investigation. No stenosis could be noted at the anastomosis at 3 years after operation as demonstration through IVU.Conclusions Retroperitoneal laparoscopic dismembered Anderson-Hynes pyeloplasty is a safe and effective way to treat the UPJO while providing a minimally invasive procedure resulting in less morbidity, better cosmetics and shorter convalescence.
