原发性醛固酮增多症52例临床分析

目的 提高对原发性醛固酮增多症(PA)的诊断和治疗水平.方法 对近5年临床确诊的52例PA患者的生化检查、体位试验、影像学检查与术后病理结果进行分析.结果 B超阳性率61.5%(24/39),CT阳性率92.3%(24/26),MRI阳性率90.9%(30/33).52例行体位试验均阳性,29例手术患者中,腺瘤17例,增生12例.醛固酮瘤(APA)患者手术后大部分效果良好,12例特发性醛固酮增多症者(IHA)术后效果良好占33%(4/12).结论 体位试验、生化检查、影像学检查联合有助于PA的定性、分型诊断,手术是PA的主要治疗方法.

Diagnosis and treatment of primary aldosteronism (A report of 52 cases)

Objective To improve the diagnosis and treatment of primary aldosteronism(PA).Methods From 1998 to 2003,the data of 52 patients with PA who were treated in our hospital were retrospectively analyzed.Results The positive rates detected by B ultrasonography,CT and MI were 61.5%,92.3% and 90.9% respectively.Among 29 patients who under went the surgery,there were 17 patients with adrenal adenomas and 12 patients with adrenocortical hyperplasia.The majority of patients who had adrenal adenomas recovered well after the surgery,the therapeutic effect after surgery in 4 patients with adrenocortical hyperplasia was satisfactory too.Conclusion Combination of posture-test,chemistry test and imaging test can contribute to the diagnosis of PA,and surgery is main thapy for PA.
