肝外胆管癌的超声诊断价值

目的探讨肝外胆管癌的超声诊断价值及漏误诊原因。方法结合手术病理结果，分析肝外胆管癌的声像图特点，总结其超声漏误诊原因。结果86例肝外胆管癌，位于肝门部39例，其中肿块型25例，非肿块型14例；超声诊断正确36例，诊断正确率为92．3％。位于胆总管47例，其中胆管逐渐变窄型21例，管壁不规则增厚型18例，肿块型8例；超声诊断正确41例，诊断正确率为89．3％。超声诊断肝外胆管癌共77例，诊断正确率为89．5％，漏误诊9例，漏误诊率为10．5％。结论超声能较准确地判断胆管癌的形态、大小及周围淋巴结肿大等异常回声，对治疗方案的制定有一定意义。

Clinical value of ultrasound in diagnosis of extrahepatic cholangiocarcinoma

Objective To explore the clinical value of ultrasound in diagnosis of extrahepatic cholangiocarcinoma,and to analyze its missed diagnosis and misdiagnosis factors. Methods Eighty-six cases of extrahepatic cholangiocarcinoma were enrolled in this study ,then their sonogram features were analyzed combined with surgical pathological results retrospectively ,and their missed diagnosis and misdiagnosis factors were summarized. Results There were 39 cases located in hilus hepatis, including 25 of mass type and 14 of non - mass type. Among these patients, 36 were accurately diagnosed by ultrasound with the diagnostic rate of 92.3 % （36/39） . There were 47 cases located in common bile duct, including 21 cases of tapering bile duct, 18 cases of irregular thickening of the tubal wall and 8 cases of mass type. Among these patients, 41 were accurately diagnosed by ultrasound with the diagnostic rate of 89.3% （41/47）. In brief, 77 cases of extrahepatic cholangiocarcinoma were diagnosed by ultrasound with the diagnostic rate of 89.5% （77/86）, 9 cases were missed diagnosed or misdiagnosed with the rate of 10.5%. Conclusion Ultrasound can accurately reveal unusual images of cholangiocarcinoma, including its shape, size and peripheral tymphadenectasis, and it has great significance in making clinical treatment plan.
