小儿特发性血小板减少性紫癜与病毒感染的关系

目的:探讨小儿特发性血小板减少性紫癜(ITP)与多种病毒感染的关系.方法:用ELISA技术对102例诊断明确的ITP患儿进行人细小病毒(HPVB19),柯萨基病毒(CBV1-6),人类巨细胞病毒(HCMV)及EB病毒(EBV)特异性IgM的检测.结果:102例ITP中HPVB19-IgM阳性41例(40.12%);CBV1-6-IgM阳性28例(27.45%);HCMV-IgM阳性17例(16.66%);EBV-IgM阳性7例(6.86%);四种病毒总阳性率50.98%,除EBV外其他三种病毒感染阳性率均明显高于正常对照组,两组比较有显著性差异(P＜0.05).结论:(1)病毒感染是小儿ITP的主要致病因素;(2)HPVB19感染是引起ITP最常见的病因;(3)HCMV感染与婴儿ITP的发生、治疗效果及预后有着明确的关系;(4)CBV1-6感染也是ITP的主要致病因素之一.

Relationship between idiopathic thrombocytopenic purpura and virus infection in children

Objective:To study the relationship between idiopathic thrombocytopenic purpura (ITP) and various virus infections in children.Methods:The human parvovirusB 19(HPV-B 19),Epstein-Barr virus(EBV),Cytomegalovirus and Coxsackie virus (CBV 1-6) were detected using ELISA test in 102 patients with ITP.Results:The HPVB-19 IgM was positive in 41 patients(40.12%).CBV1-6 IgM was positive in 28 patients(27.45%).The CMV IgM was positive in 17 patients(16.66%).The EBV IgM was positive in 7 patients(6.86% ).The total rate of these four viruses was 50.98% of the patients with ITP observed in this study.Except EBV,the virus infections positive rate in this study were significantly higher than that of control group.( P <0.05).Conclusion:(1)These results suggest that the pathogenesis of ITP in childhood might be correlated with virus infections.(2)HPV-B19 infecteion is the most common virus in the patients with ITP.(3)The CMV infection is associated with the treatment effect and prognosis clinically in infant with ITP.(4)The CBV1-6 infection may be one of the virus caused ITP.
