三种途径子宫切除术的临床效果比较

目的比较经腹子宫切除术(TAH)腹腔镜辅助阴道子宫切除术(LAVH)和阴式子宫切除术(TVH)三种手术方式的临床效果。方法对483例因妇科良性疾病需行子宫切除患者做一回顾性分析,其中216例行TVH,121例行TAH,101例行LAVH。对三组手术情况、手术费用、术后恢复情况进行比较。结果三种手术方法在手术时间、术中出血、术后恢复、手术费用、住院天数差异均有统计学意义(P<0.05)。结论各种术式的子宫切除术各具有其利弊和相应的适应证。在决定手术方式时应根据患者的病情考虑各种术式的利弊,避免并发症的发生是手术成功的关键。

Clinical Comparison between Three Methods of Hysterectomy Operations

Objective To compare the clinical effect between Total abdominal hysterectomy(TAV),laparoscopically assisted vaginal hysterectomy(LAVH)and transvaginal hysterectomy(TVH).Methods A retrospective survey was made on 483 patients who suffered from gynecological diseases and need hysterectomy.216 patients were performed TVH,121 patients were performed TAH,101 patients were performed LAVH.The effects of operation,cost and recovery of 3 groups were compared.Results There were significant differences in the operation time,bleeding during operation,recoveries after operating,cost of operation and the days of hospitalization among 3 groups(P<0.05).Conclusion Different surgical patterns of hysterectomy have their distinctive advantages.To make a right decision for the patient who has the indication for hysterectomy,the surgeon should comprehensively consider the advantages and disadvantages of various patterns according to the patients'conditions.The key point of a successful operation is to avoid complications of the operation.
