临床路径在喉癌患者围手术期肠内营养支持中的应用

目的 探索喉癌患者围手术期肠内营养支持护理对策.方法 选择28例喉癌手术住院患者,按照临床路径的工作方法,采用成品和自制匀浆肠内营养膳通过鼻胃管注入,使用体重指数(BMI)、血液生化指标对28例患者在术前1 d、术后14 d进行营养评估,比较术前与术后各项指标的差异.结果 28例患者BMI营养评价,术前1 d、术后14 d轻度营养不良率分别为10.7%和14.3%;按照临床路径方法鼻饲成品和自制匀浆肠内营养膳,术后14 d与术前1 d的血红蛋白、血尿素氮水平比较差异显著,而BMI、总淋巴细胞计数、血清白蛋白、血清总蛋白之间比较无显著差异.结论 按照临床路径的工作方法、使用配比合理的匀浆膳是喉癌患者围手术期肠内营养支持的一种比较科学和值得推广的护理对策.

Application of clinical pathway in perioperative nutritional support of laryngeal cancer patients

Objective To explore the perioperative nutritional support and nursing countermeasures for laryngeal cancer patients. Methods 28 cases of hospitalized laryngeal surgery patients, in accordance with the working method of clinical pathway, finished product and homemade homogenate enteral nutrition was fed through nasogastric tube. Nutrition evaluation was finished by use of body mass index (BMI), blood biochemical indicators on 1d preoperation and 14 d postoperation, All indexes were compared between preoperation and postoperation. Results Nutrition assessment with BMI showed incidence of mild malnutrition as 10.7% and 14.3% on 1d preoperation and 14 d postoperation. Nutrition evaluation index such as HB, BUN was statistically significant, and BMI、 TLC、ALB、TP showed no statistical sisnificant difference between 1d preoperation and 14 d postoperation. Conclusions Clinical pathway in accordance with the working method, using the reasonable meal is laryngeal cancer patients homogenate perioperative enteral nutrition support a more scientific and deserves popularization of nursing countermeasures.
