改良冷刀在腹腔镜子宫腺肌瘤剔除术中的应用研究

目的探讨改良冷刀在腹腔镜子宫腺肌瘤剔除术的可行性和临床效果。方法选取2008年10月至2013年10月子宫前壁或宫底单发腺肌瘤患者22例,根据患者要求分组:观察组(腹腔镜组)10例,对照组(开腹手术组)12例,观察两组患者的手术时间、术中出血量、术后疼痛、住院时间、术后肛门排气时间及术后痛经缓解情况。结果两组患者手术时间、术中出血量比较差异无统计学意义(P>0.05)。两组患者术后疼痛、住院时间、术后肛门排气时间均较开腹组的患者短,疼痛程度轻,差异有统计学意义(P<0.05)。两组患者术后痛经均明显缓解,差异无统计学意义(P>0.05)。结论改良冷刀应用在腹腔镜子宫腺肌瘤手术中疗效确切,患者创伤小、术后恢复快,是一种安全、微创、可行的手术治疗方法。

Application of Improved Cold Sword in the Laparoscopic Myomectomy in Patients with Uterine Gland

Objective To investigate the feasibility and clinical effects of improvement of cold sword in the laparoscopic myomectomy for uterine gland. Methods 22 patients with the anterior wall of the uterus or uterine fundus gland leiomyoma were slected from October 2010 to October 2013. According to the requirement of patients, all the patients were divided into observation group （laparoscopic group） and control group （open group）. The observation group includes10 cases, the control group includes 12 cases. The operation time, operation in two groups of patients with bleeding, postoperative pain, hospitalization time, postoperative anal exhaust time and postoperative pain relief were observed. Results There was no significance difference about operation time, intraoperative bleeding volume between two groups （P〈0.05）. The postoperative pain, hospitalization time, postoperative anal exhaust time of observation group was better than that of the control group, and there was significance difference between two groups. The postoperative pain of two groups were all were relieved with was, but there was no significant difference between two groups. Conclusion The efficacy of improved cold sword in laparoscopic uterine myoma operation is exactly clear, and the operation was showed as fewer traumas, less complication and rapid recuperation, so it is a safe, feasible, minimally invasive operation treatment.
