腰椎管狭窄症中脊髓降钙素基因相关肽的表达

摘    要:背景:降钙素基因相关肽是神经元细胞合成并分泌的感觉神经递质,由轴浆转运或血液到达靶器官,通过影响靶器官的功能以发挥激素样作用。其表达变化与腰腿痛密切相关。目的:观察腰椎管狭窄时脊髓内降钙素基因相关肽的变化。设计:随机对照动物实验。单位:中国医科大学附属第一医院骨科。材料:实验于2003-01/2004-12在中国医科大学完成。选择成年雄性SD大鼠30只,按照取材时间(2,4,8,12周)的不同分为4组,另有1组对照组。每组6只。方法:制备腰椎管狭窄模型,并采用免疫组化方法检测不同时间压迫(2,4,8,12周)后,脊髓后角降钙素基因相关肽的表达。光镜下以神经元细胞核蓝色着色,细胞浆出现棕褐色颗粒定为免疫组化染色阳性。每例切片在脊髓横断面随机选取5个高倍视野,计数阳性细胞数,主要观察指标:脊髓后角中降钙素基因相关肽的表达。结果:30只动物实验过程中无死亡,全部进入结果分析。随着马尾神经受压时间的延长,脊髓后角降钙素基因相关肽的含量有不同程度的增加,手术后2,4,8,12周后脊髓后角降钙素基因相关肽阳性细胞数目依次为(1.02±0.15),(4.28±0.22),(2.11±0.21),(3.03±0.25),均显著高于对照组(0.66±0.12),差异有显著性(t=2.01-5.56,P<0.05)。结论:腰椎管狭窄症中马尾神经受压后,脊髓降钙素基因相关肽的表达增高。

Abstract:BACKGROUND: Calcitonin gene-related peptide (CGRP), as a neurotransmitter synthesized and secreted by neurons, reaches the target organ through axoplasmic transport or blood and exerts an estrogen-like function through affecting the function of the target organ. Its expression is closely related to lumbocrural pain.OBJECTIVE: To observe the changes of CGRP in spinal cord when lumbar stenosis occurs.DESIGN: A randomized controlled animal experiment.SETTING: The Department of Orthopaedics of the First Hospital Affiliated to China Medical University.MATERIALS: This experiment was conducted at China Medical University between January 2003 and December 2004. Totally 30 adult male SD rats were chosen. Animals were divided into 4 groups according to different times for sampling (weeks 2, 4, 8 and 12) and another group as the control,with 6 rats in each one.METHODS: Lumbar spinal stenosis models were prepared; the expressions of CGRP of spinal cord posterior horn compressed at different time were detected with immunohistochemical method. Under the light microscope, when the nucleus of the neurons was colored blue and brown particles appeared in the cytoplasm, it would suggest that immunohistological staining showed positive. 5 high-fold fields were randomly chosen at the spinal cross section of each slice and the positive cells were counted.MAIN OUTCOME MEASURES: The expressions of CGRP of spinalcord posterior hornRESULTS: All the animals survived the experiment and all entered the stage of result analysis. With the prolonging of the time for compression on the cauda equina, the level of CGRP of spinal cord posterior horn increased at different degrees. The number of positive cells of CGRP of spinal cord posterior horn at postoperative week 2, 4, 8 and 12 was (1.02±0.15), (4.28±0.22), (2.11±0.21), (3.03±0.25) respectively, which was all significantly higher as compared with the control group (0.66±0.12),with a significant difference(t=2.01-5.56,P < 0.05).CONCLUSION: In lumbar spinal stenosis, the expression of CGRP increased when the cauda equina was compressed.
