布托啡诺在膝关节镜术后镇痛中的临床应用

目的：探讨布托啡诺在膝关节镜术后关节腔内的镇痛效果。方法：选择40例ASAI-II级在腰硬联合麻醉下行膝关节镜手术的患者,随机分为对照组和布托啡诺组,每组20例。对照组在术毕关节腔内注射0.5%布比卡因5ml、生理盐水5mL;布托啡诺组则注射布比卡因5ml、布托啡诺2mg,两组均用生理盐水稀释至10ml。观察术后4、8、12、24小时的VAS评分和呼吸抑制、恶心呕吐、尿潴留和皮肤瘙痒等不良反应。结果：与对照组比较,布托啡诺组的4、8、12、24小时的VAS评分显著降低,P〈0.05;两组术后均无不良反应发生。结论：关节腔内注射布托啡诺能够明显缓解膝关节镜手术的术后疼痛。

Clinical application of butorphanol for pain relief after knee arthroscopy

Objective：To explore the analgesia effect of butorphanol in the joint cavity after knee arthroscopy. Methods： 40 cases of ASAI - II level with lumbar epidural anesthesia for knee arthroscopy were randomly divided into control and butorphanol groups of 20 patients. The control group received intra-articular injection of 0.5% bupivacaine 5ml and 0.9%NS 5ml and butorphanol group received intra-articular injection of bupivacaine 5ml and butorphanol 2mg, in which two groups were diluted with NS to 10ml. VAS scores, respiratory depression, nausea and vomiting, urinary retention, skin pruritus and other adverse reactions were observed after 4,8,12,24 hours of surgery. Results：Compared with the control group, VAS score in butorphanol group of 4,8,12,24 hour were significantly low（P〈0.05）;and both groups had no adverse reactions. Conclusion：Intra-articular injection of butorphanol can significantly improve postoperative pain of knee arthroscopy.
