摘    要:目的评价符合线路18FDG-SPECT(18FDG-SPECT with coincidence)在鉴别肺部良恶性病变中的作用.方法对86例胸部X线片或CT异常的患者进行18FDG-SPECT检查.18FDG-SPECT扫描结果经两位以上有经验的核医学科医生阅片进行定性分析,同时采用肺部病变/正常肺组织的18FDG摄取比值(T/N)对病变的良恶性进行鉴别.以病理学诊断作为金标准,SPECT扫描结果均与其进行比较.结果86例肺部病变中56例为恶性,30例为良性.对照术后病理检查结果18FDG-SPECT诊断的准确性、敏感性、特异性、阳性预测率和阴性预测率分别为94.2%,96.4%,90%,94.7%和93.1%.结论符合线路18FDG-SPECT检查能准确地鉴别肺部病变的良恶性,其诊断的准确性与18FDG-PET扫描相似,但与18FDG-PET相比,18FDG-SPECT具有价廉且易于开展的优点.

Abstract:Objective: To evaluate the ability of 18FDG-SPECT with coincidence to differentiate benign from malignant lung lesions. Methods: Within 4 years 86 patients with abnormal chest radiograph underwent 18FDG-SPECT. 18FDG-SPECT interpretation was based on visual qualitative analysis of images. Ratio of 18FDG activity of tumor over normal lung(T/N)assessed visually was used as criteria for differentiation between benign and malignant lesions. 18FDG-SPECT results were compared with pathological results, which were the gold standard. Results: Of 86 lung lesions, there were 56 malignant and 30 benign diagnoses. Compared with pathological results, the accuracy, sensitivity, specificity, positive predictive value and negative predictive value of 18FDG-SPECT were 94.2%,96.4%,90%,94.7% and 93.1% respectively, for detecting malignant lung lesions. Conclusions: 18FDG-SPECT with coincidence imaging accurately differentiates malignant from benign lung lesions which is comparable with prior 18FDG-PET results. 18FDG-SPECT with coincidence appears promising as a more easily available and less expensive technique than 18FDG-PET.
