| 摘 要： | 阐述了充分利用信息技术和网络通信技术手段，建立首都医科大学附属复兴医院与月坛社区卫生服务信息共享网络平台、双向转诊信息平台及与居民家庭互动信息平台，并依托三个平台实现医院与社区双向转诊、健康信息咨询、居民电子健康档案信息、家庭信息互动等医院与社区的新型卫生协同服务模式。

| Abstract: | It expatiates that the network platform of health service information sharing, information platform of two-way transfer of patients and household interaction information platform between Fu Xing Hospital of Capital Medical University and Yuetan Community are constructed by full use of information technology and network communication technology means; besides, it relies on these three platforms to realize the new health cooperative service modes for hospital and community, such as the two-way transfer of patients, health information query, resident electronic health files information, and family information interaction, etc. |
