甲状腺功能减退性心脏病的超声心动图改变

摘    要:目的 研究甲状腺功能减退性心脏病的超声改变,并探讨超声心动图对甲状腺功能减退性心脏病的诊断价值。方法 以42例甲状腺功能减退性心脏病的超声心动图改变与正常人对比。结果 42例甲状腺功能减退性心脏病忠者中有心包积液28例。发生率67%。心脏扩大18例。占43%;左室壁轻度增厚22例;左室射血分数减低20例,占48%。42例患者心动周期时相中左室射血前期时间(PEP)延长,左室射血前期时间/射血时间(PEP/ET)之比上升,与正常对照组相比有显著性差异。结论 彩色多谱勒超声心动图对甲状腺功能减退性心脏病的诊断与鉴别诊断有重要意义,其中PEP/ET比值上升是超声心动图诊断甲状腺功能减退性心脏病的敏感指标。

Abstract:Objective To study the ecocardiographic changes of hypothyroidism cardiopathy and inquire into the diagnostic value of echocardiography in identifying hypothyroidism cardiopathy.Methods The comparison studies of the ecocardiographic changes were conducted between both the 42 cases of cardiopathy of hypothyroidism and the normal ones.Results Among the total 42 cases of cardiopathy of hypothyroidism, there were 28 cases of pericardial effusion with the rate of occurrence being 66 per cent,18 cases of cardiomegaly, making up 42 per cent, 22 cases were as the slight thickening of left ventricular wall, and 20 cases of low ejection fracture of left ventricle, accounting for 48 per cent of the total.For the time periods of the 42 cases the pre-ejection period (PEP) got longer and the rate of pre-ejection period to the ejection time went up as well, it was of significant difference in comparison with the normal control group.Conclusion The Doppler Ecocardiography was of great importance in diagnosing and differential diagnosing of hypothyroidism cardiopathy, of which the going up of the rate of PEP/ET was the sensitive index of ecocardiographic diagnosis of hypothyroidism cardiopathy.
