摘    要:目的探讨持续输注异丙酚对兔肝静脉血氧饱和度和氧供平衡的影响。方法30只成年健康兔随机分为异丙酚组(P组,n=15)和脂肪乳组(C组,n=15)。分别输注0.6mg/(kg·min)异丙酚或10%脂肪乳。记录给药前(t0)和后10(t10)、30(t30)、60(t60)min两组的肝静脉血氧饱和度、肝动脉、肝静脉、门静脉的血流量,计算肝氧供、氧耗。结果持续输注10%脂肪乳对肝血流、肝氧代谢的影响差异无统计学意义。持续输注脂溶性异丙酚,肝静脉血气饱和度随输注时间的延长差异无统计学意义;肝门静脉血流、肝血流随着输注时间的延长而增加,肝脏氧供与氧耗均增加;兔肝氧供/氧耗随榆注时间的延长差异无统计学意义。结论持续输注异丙酚肝静脉血氧饱和度无降低,兔肝氧供/氧耗不变。

Abstract:Objective To investigated the effect of propefol on hepatic blood flow and the hepatic vein oxygen saturation (ShvO2)in rabbits,establish its net effect on hepatic oxygen balance. Methods Thirty adult males New Zealand white rabbits were randomly divided into two groups: Group P propofol,0. 6mg/( kg · min)] or Group C 10% intralipid, 0. 6mg/( kg ·min) ]. An electromagnetic flowmeter was used to measure hepatic blood flow, and blood, from the carotid artery, the portal vein, and the hepatic vein, was used to determine hepatic oxygenation. After we obtained baseline values, we repeated measurements 10, 30 and 60min after initiating the infusion. Results Intralipid did not affect systemic hemedynamics,hepatic blood flow,or oxygenation during the 60min infusion;however, propofol caused a time-dependent decrease in mean arterial blood pressures and an increase in portal venous flow and total hepatic blood flow. In contrast, hepatic arterial blood flow remained unchanged during the propofol infusion. Hepatic oxygen delivery and consumption increased in a time-dependent manner to maximums of 25% and 21.4% (both,P〈0. 05 ) above baseline,respectively. Hepatic venous oxygen saturation and extraction was unchanged throughout the study period. Conclusion Propefol increases total hepatic blood flow,primarily by increasing hepatic portal venous flow. The increase in liver oxygen consumption was fully compensated by an increase in oxygen supply to the liver, resulting in a preserved, hepatic oxygen balance. The results indicated that the hepatic vein oxygen saturation and the ERO2 were unchanged throughout the propefol infusion period.
