羊水中甲状腺激素检测及其临床应用

摘    要:采用体外放射分析方法对22例正常和30例正在治疗的患甲状腺疾病的孕妇羊水T3、T4、TSH、rT3及FT3、FT4进行了测定。并监测孕妇妊娠期甲状腺功能和进行了新生儿甲低筛查。结果表明羊水中甲状腺激素水平的测定,对了解母体服药对胎儿的影响及有无胎儿甲低是有价值的。羊水中rT3水平的测定可作为观察及筛选胎儿甲低的方法之一。

Abstract:The amniotic fluid concentrations of T3.T4.TSH.rT3 and FT3,FT4 were determined in 22normal pregnant women and 30 preganant patients with thyroid diseases under treatment during 19-22 weeks with radioimmunoassay. Maternal serum during gestation and cord serum concentrations of thyroid hormones were also monitored. Newborns were screened for congenital hypothyroidism. The results showed that there were no significant differences between the amniotic fluid concentrations of thyroid hormones in the normal and disease-treated groups. During gestation, 30 disease-treated patients serum thyroid hormones levels remained within normal range. In 28 specimens, the average umbilical cord serum T3 and FT3 concentrations were lower than average, but T4 and FT4 concentrations were normal in 20 of these the TSH and rT3 concentrations were also higher. In 3 cases of 30 newborns TSH values of blood spot disks were higher than normal (<25mU/L), however, serum thyroid function tests returned to normal at follow-up test after 12 days. It is suggested that determining the amniotic fluid concentrations of thyroid hormones is of value for detecting the maternal drugs influence on fetal thyroid function, of wich the determination of amniotic fluid rT3 level serves best and may be applied to screen fetal hypothyroidism.
