1990—2019年江苏省2型糖尿病所致慢性肾病疾病负担及变化趋势分析

目的 分析和比较1990—2019年江苏省2型糖尿病所致慢性肾病疾病负担及其变化趋势，为江苏省慢性肾病的预防控制和策略制定提供理论依据。方法 从2019年全球疾病负担研究中获取江苏省2型糖尿病所致慢性肾病疾病负担指标，分析和比较1990—2019年2型糖尿病所致慢性肾病发病率、患病率、死亡率、伤残调整生命年(disability adjustedlife years, DALY)、早死损失寿命年(years of lost life, YLL)和伤残损失健康寿命年(years lived with disability, YLD)等指标，分析江苏省2型糖尿病所致慢性肾病疾病负担及其变化趋势。结果 2019年江苏省2型糖尿病所致慢性肾病发病率、患病率和死亡率分别为32.42/10万、2 355.47/10万和3.37/10万，与1990年相比标化发病率和患病率增长了3.95%和3.71%,死亡率下降了23.49%。2019年江苏省2型糖尿病所致慢性肾病DALY率、YLL率和YLD率分别为88.84/10万、61.29/10万和27.55/10万。与1990年相比，标化DALY率和YLL率...

Burden and its changing trend of chronic kidney disease due to type 2 diabetes mellitus in Jiangsu Province, 1990-2019

Objective To analyze and compare the burden of chronic kidney disease due to type 2 diabetes mellitus (CKD-T2DM) and its changing trend in Jiangsu Province from 1990 to 2019, and to provide a theoretical basis for prevention and control of chronic kidney disease and formulation of strategies in Jiangsu Province. Methods Data regarding disease burden of CKD-T2DM in Jiangsu Province from 1990 to 2019 were obtained from the Global Burden of Disease Study 2019. The incidence, prevalence, mortality, disability-adjusted life years (DALY), years of lost life (YLL), years lived with disability (YLD) and other indicators of CKD-T2DM from 1990 to 2019 were analyzed and compared. Results The incidence, prevalence and mortality of CKD-T2DM in Jiangsu Province in 2019 were 14.91/100,000, 1,539.52/100,000 and 1.87/100,000, respectively. Compared with 1990, the age-standardized incidence and prevalence increased by 3.95% and 3.71%, respectively, while the mortality dropped by 23.49%. The DALY rate, YLL rate and YLD rate of CKD-T2DM in Jiangsu Province in 2019 were 88.84/100,000, 61.29/100,000 and 27.55/100,000, respectively. Compared with 1990, the age-standardized DALY and YLL rates decreased by 21.68% and 30.88%, respectively, but the YLD rate increased by 12.31%. The DALY and YLL rates were higher in males than in females (61.46/100,000 vs. 49.56/100,000, 45.39/100,000 vs. 32.41/100,000), but the YLD rate was higher in females than in males (17.15/100,000 vs. 16.07/100,000). The DALY, YLL and YLD rates increased with age, and were found to be the highest in the group aged 70 years and above. Joinpoint analysis displayed that the age-standardized DALY rate of CKD-T2DM in Jiangsu Province from 1990 to 2019 showed a declining trend (AAPC=-0.79, t=-3.339, P<0.001). Conclusion The burden of CKD-T2DM in Jiangsu Province from 1990 to 2019 showed a downward trend, mainly manifested by the burden of premature death. The overall disease burden of male and elderly patients is high. Hence comprehensive prevention and treatment should be focused on male and elderly patients with T2DM so as to reduce the disease burden of CKD-T2DM.
