血浆基质金属蛋白酶在脑梗死患者的动态变化及其与神经功能评分的相关性

摘    要:目的:探讨脑梗死患者血浆基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平的动态变化及其与神经损害的关系。方法:50例对照组与179例脑梗死患者发病24h内、第3天和第14天采血,用酶谱法测定MMP-2和MMP-9,比较其差别,并分析血浆MMP-2和MMP-9水平动态变化与脑梗死患者神经损害的关系。结果:①179例脑梗死患者发病24h内、第3天时血浆MMP-2、MMP-9浓度水平均高于正常对照组。第3天血浆MMP-2和MMP-9水平最高(P<0.05)。②发病24h内、3d、14d血浆MMP-2及MMP-9水平变化与脑梗塞患者神经功能缺损程度有关,神经功能缺损程度越重,血浆MMP-2及MMP-9水平越高,差异有显著性(P<0.05)。③第3天MMP-2、MMP-9水平之间显著相关(r=0.751),MMP-9与神经功能缺损程度评分的相关系数(r=-0.463,P<0.05)强于MMP-2(r=-0.218,P<0.05)。结论:MMP-2和MMP-9参与脑梗死的病理过程,脑梗死急性期血浆MMP-2、MMP-9水平可作为判断脑梗死神经功能缺失、病情轻重及预后的客观指标。

Abstract:Objective :To observe the dynamic changes of metalloproteinase-2(MMP-2) and metallopro-teinase-9(MMP-9) in patients with cerebral infarction .To observe the relation of MMP-2、MMP-9 levels to Neuronal Damage Score ,NIHSS (National Institute of Health stroke scale ) on the time course in ischemic stroke .Methods :MM Ps in plasma was measured in 179 patients with acute cerebral infarction (ACI) and 50 controls on the first 24 hour ,third day and 14th day .At the same time ,NIHSS (National Institute of Health stroke scale ) were also ob-tained and analyzed .Results :① Plasma MMP-2、MM P-9 in 179 patients significantly elevated than in controls on the first 24h and the third day ,(P&lt;0 .05) .plasma MMP-2 on third day and plasma MM P-9 on the first 24 h was highest among these .②Moreover ,Mean plasma MMP-2、MMP-9 activities in patients NIHSS &gt;15 were significantly higher than those in the patients with NIHSS≤15(P&lt;0 .05) .③On third day MMP-2 activities was significantly correlated with MM P-9 in plasma(r=0 .751 ,P&lt;0 .05) .Correlation of MMP-9 with NIHSS(r=-0 .463 ,P&lt;0 .05)more signifi-cantly than MMP-2(r=-0 .218 ,P&lt;0 .05) .Conclusion:MMP-2、MM P-9 are involved in the pathologic process of cer-ebral infarction .The concentrations of MMP-2 and MMP-9 may be taken as an objective guide to judge the neuronal damage recovery and the recent prognosis in acute cerebral infarction .
