真菌性角膜溃疡角膜移植术后免疫抑制剂和糖皮质激素的应用

摘    要:目的:观察真菌性角膜溃疡角膜移植术后应用免疫抑制剂和糖皮质激素的疗效和最佳应用时间。方法:54例真菌性角膜溃疡患者行穿透角膜移植术后随机分为两组,1组术后用大肤康滴眼液,术后2周开始加用环孢霉素A滴眼液和全身使用糖皮质激素,2组术后只用大扶康滴眼液,出现排斥后加用免疫抑制剂和糖皮质激素。结果:两组比较:1组前房反应轻、排斥反应发生率低且症状轻,真菌复发率未见增高。结论:真菌性角膜溃疡角膜移植术后,联合应用免疫抑制剂和糖皮质激素,疗效好。

Abstract:ObjectiveTo observe the effect and the best time of the administration of immunosuppressive drug and corticosteroid after penetrating keratoplasty(PKP)in management of fungal corneal ulcer.Methods54 cases of fungal corneal ulcer were separated into two groups.Doflucan drops were administered in group I after transplantation,CsA drop and corticosteroid(used in the whole body)were added after 2 weeks post-operation.Doflecan drops were administered only in group Ⅱ.If rejection reaction occur, immunosuppressive drug and corticosteroid were added.ResultsRelapse rate of fungal corneal ulcer in group Ⅰ didn’t increase and the inflammatory reaction in anterior chamber was less serious than group Ⅱ.ConclusionIt is very effective for alleviating the inflammatory reaction and reducing the rejection rate after PKP if immunosuppressive drug and corticosteroid are administered reasonably.
