颈椎间盘髓核组织中TRAIL及DR5的表达与髓核细胞凋亡的相关性研究

摘    要:目的检测颈椎间盘内髓核组织细胞密度、细胞凋亡率以及肿瘤坏死因子相关凋亡诱导配体(TRAIL)及死亡受体5(DR5)的表达。方法实验组30个标本来自脊髓型颈椎病患者前路手术所切取的椎间盘组织(C3-C7),对照组30个标本为颈椎创伤患者前路手术所取得的椎间盘组织。采用HE染色观察髓核组织中凋亡小体和细胞密度,原位DNA片段末端标记法(TUNEL)检测凋亡细胞率,用免疫组织化学法检测TRAIL、DR5的表达。结果实验组髓核组织中的细胞密度均低于对照组,TUNEL阳性细胞率均高于对照组(P〈0.01);实验组TRAIL、DR5染色阳性细胞率均高于对照组(P〈0.05)。将两组全部60例颈椎间盘标本合并后进行Pearson相关分析,颈椎间盘TUNEL阳性细胞率与细胞密度之间呈负相关(r=-0.969,P〈0.01);两组髓核内TRAIL、DR5染色阳性细胞率与TUNEL阳性细胞率之间呈正相关r=0.921和0.755,P〈0.01)。结论细胞密度下降参与了颈椎间盘的退变过程,细胞凋亡是椎间盘髓核细胞减少的原因之一,颈椎间盘组织中存在DR5/TRAIL诱导的凋亡途径。
