小剂量卡托普利、络沙坦治疗心力衰竭的临床观察

目的 比较单用常规剂量血管紧张素转换酶抑制剂（ACEI）卡托普利与合用小剂量卡托普利，血管紧张素Ⅱ受体Ⅰ拮抗剂（ARB）络沙坦治疗心力衰竭（HF）的疗效。方法 选择住院的HF患者84例，随机分成2组，在常规治疗的基础上，分别加服卡托普利12.5mg id，和卡找普利6.5mg tid，络沙坦25md qd治疗10周，观察治疗前后左室舒张末内径（LVEDD），左室射血分数（EF），A峰值速度（A峰），E峰值速度（E峰），峰值速度比（A／E），血浆肾素活性（PRA），血管紧张素Ⅱ（AngⅡ），心钠素（ANP），内皮素（ET）的变化。结果 二药合用组EF值的提高，A／E，ET值的降低较单用ACEI组有显著的差异，少见副作用。结论 ACEI，ARB合用治疗HF疗效不仅优于ACEI潼，而且是一种安全的抗心衰药物组合。

Effect of low- dose combined captopril and losartan therapy on heart failure

Objective: To assess the effect of low-dose combined captopril and losartan on heart failure. Methods: Eight four patients with heart failure were divided into 2 groups randomly. Group A received aptopril 12.5mg tid and routine therapy and group B used captopril 6.25 mg tid, losartan 25mg qd and routine therapy for 10 weeks. Results: There were significant changes of EF, A/E and the plasma levels of ET. Conclusion: The effect of low-dose ACEI and ARB combination on heart failure is better than ACEI, and it is safe.
