尿激酶、肝素联合治疗兔脑栓塞的实验研究

摘    要:目的探讨栓塞后3 h尿激酶、肝素联合治疗兔脑栓塞的有效性和安全性。方法自体血栓栓塞后3h,将30只兔随机分为3组,尿激酶(2.0万U/kg)组、尿激酶 肝素(2 000 U,1次/6 h)组、生理盐水组。测定治疗前后活化部分凝血活酶时间(activeted partial thrombopla stin time,APTT),评定栓塞后24 h神经功能并计算梗死体积。结果与生理盐水相比,使用尿激酶、尿激酶 肝素均可减少梗死体积、减轻神经功能损伤。尿激酶 肝素治疗后APTT大于120 s。但3组出血发生率无明显差别。结论兔脑栓塞后3 h联合尿激酶和肝素治疗可显著减小梗死体积,明显改善神经功能,且未增加脑出血的危险。

Abstract:Objective To evaluate the efficacy and safety of administration of intravenous urokinase(UK) combined with heparin on acute cerebral embolism in rabbit.Methods Thirty rabbits with autologus thrambas embolism throagh internal carotid artery were randomized divided into three groups,UK(20 000 U/kg) group,UK plus heparin(2 000 U per six hours) group and saline group.Activeted partial thrombopla stin time(APTT) was measured before and after treatment.Twenty-four hours after embolism,the neurological function was assessed and infarct volume was determined using a computer image analyzer.Results It had been shown that infarct volume was reduced and neurological dysfunction was improved in the UK and UK plus heparin group when compared with the saline group.In those animals that received UK plus heparin,the APTT was maintained at above 120 seconds,but there was no statistically significant difference in the incidence of hemorrhage among the three groups.Conclusion Administration of UK combined with heparin could significantly reduce the infarct volume and improve the neurological dysfunctions.Moreover this treatment did not increase the risk of hemorrhage.
