前额纵裂入路切除儿童鞍区肿瘤100例

目的 探索前额纵裂入路切除儿童鞍区肿瘤的适应证，方法及优缺点。方法回顾分析北京天坛医院神经外科小儿组1997年5月至2001年5月经前额纵裂入路切除鞍区肿瘤100例。结果 肿瘤全切除或近全切除87例，大部分切除13例；其中双侧嗅神经保留84例。结论 前额纵裂入路是切除向鞍抽，鞍上及三脑室前部生长肿瘤比较好的方法，手术在直视下操作，手术盲区小，便于保留重要结构。肿瘤全切率高。

100 case of pediatric sellar region tumors treated with the anterior interhemispheric approach

Objective To study the indications, methods, advantages and disadvantages of anterior interhemispheric approach for removing pediatric tumors in sellar region.Methods 100 cases of anterior interhemispheric approach surgery for pediatric sellar region tumors from May, 1997 to May, 2001 were analyzed retrospectively. Results The tumors were totally or subtotally removed in 87 cases, and partially removed in 13 cases. In 84 cases the olfactory nerve were successfully preserved bilaterally, and in 81 cases the pituitary stalk were anatomically preserved.Conclusions Anterior interhemispheric approach is a good choice for tumors located in the sellar region and anterior part of third ventricle.The tumor could be removed with high percentage of total resection under the direct exposure and the important anatomic structures could be easily preserved.
