顺铂联合长春花硷胸腔内注射对恶性胸腔积液的疗效观察

目的 为探讨胸腔内注入冻干顺铂(DDP)联合长春花硷(VLB)对恶性胸腔积液的治疗效果。 方法 病理确诊的恶性胸腔积液50例,经胸穿抽尽胸液后,随机分为治疗组(27例)给予DDP 60mg、VLB 10mg胸腔内注射,对照组(23例)给予DDP 60mg胸腔内注射,一周后重复一次。观察其疗效、生存率、生活质量及毒副反应。 结果治疗组总有效率77.8％,病变进展率7.4％,而对照组分别为52％和26％。治疗组0.5年、1年、1.5年和2年的生存率为92.6％、55.6％、37.0％和3.1％。而对照组分别为78.3％、56.5％、21.7％和4.3％,其中0.5年和1.5年的生存率有显著性差异(P<0.05),治疗组的白细胞下降程度较对照组明显。 结论胸腔内联合注入DDP和VLB对治疗恶性胸腔积液是安全有效的。

An observation of curative effect on application of Cis-Diaminodichloroplatinum (DDP) combining with Vincaleucoblastine (VLB) by intrapleural injection to treat malignant pleural effusion

Objective To investigate the curative effect on application of DDP combining with VLB by intrapleural injec-b'on to treat malignant pleural efiusion. Methods 50 cases of malignant pleural effusion were randomly divided into two groups: the therapeutic group(27 cases) and the control group(23 cases) .The therapeutic group was given DDP60mg combining with VLB 10mg by inbapleural injectcon,the control group only DDP 60mg,and above-mentioned drugs were repeated after 1 week.The curative efiect,survival rates and quality of life as well as toxic and side reactions were observed and kept up for 2 years. Results The total effective rate and progressive rate of morbid change in the therapeutic group and the control group were 77.8% and 7.4% vs 52% and 26% respectively.As the patients of two groups were in 0.5, 1, 1.5 and 2 years, the survival rates were 92.6%,55.6%,37.0% and3.1% vs78.3%,56.5%,21.7% and4.3% ,and there were significant diflerences in the survival rates of 0.5 and 1.5 years between both groups.The WBC had a marked decrease in therapeutic group than control group. Con-clusion DDP combining with VLB by intrapleural ingection to treat malignant pleural effusion may be a safe and effective method.
