重组人粒细胞刺激因子治疗急性放射性黏膜炎的疗效观察

摘    要:目的 观察重组人粒细胞刺激因子防治急性放射性黏膜炎的疗效。方法 回顾性分析30例重组人粒细胞刺激因子治疗急性放射性黏膜炎患者,并随机抽取同期未予重组人粒细胞刺激因子治疗的30例急性放射性黏膜炎患者作为对照组。对照组:生理盐水80 ml+地塞米松5 mg+庆大霉素8万u+利多卡因400 mg(三餐后+睡前)每次20ml含漱。观察组:对照组基础上予生理盐水80 ml+欣粒生300 μg。观察重组人粒细胞刺激因子治疗后急性放射性黏膜炎疗效及转归。结果 观察组放射性黏膜炎发生率明显低于对照组,秩和检验显示,在放疗结束时,观察组3,4级放射性粘膜炎分级明显低于对照组,具有统计学差异(P < 0.05)。口腔、咽部疼痛观察组明显低于对照组,放疗第35天,放疗结束时具有统计学差异(P < 0.05)。结论 重组人粒细胞刺激因子能推迟头颈部肿瘤患者放射性口腔炎的发生时间,降低3,4级放射性黏膜炎的发生率,并能促进放射性黏膜炎的愈合,缓解患者口咽疼痛。

Abstract:Objective To evaluate the effect of recombinant human granulocyte colony-stimulating factor on acute radiation mucositis. Methods 30 cases of acute radiation mucositis treated with recombinant human granulocyte colony-stimulating factor were analysed retrospectively,and 30 cases of acute radiation mucositis without it were selected as a control group in the same period. The control group was treated by saline 80 ml + dexamethasone 5 mg + gentamicin 80000 u + lidocaine 400 mg (after meals + before bedtime) with gargling administration,20 ml each time. The observation group was treated by saline 80 ml + xinlisheng 300 μg. on the basis of the control group. The efficacy of acute radiation mucositis was observed. Results The radiation mucositis incidence of observation group was lower than that of the control group. The grade 3 and 4 radiation mucositis was significantly lower than the control group at the end of radiotherapy (P < 0.05). The in observation group, pain scores were lower than that of the control group,it was significantly differente at the radiotherapy day 35 and at the end of radiotherapy (P < 0.05). Conclusion Recombinant human granulocyte colony stimulating factor can delay the generation time of radioactive stomatitis, reduce the incidence of the grade 3 and 4 radiation mucositis,and reduce the pain,promote healing of radiation mucositis.
