2型糖尿病患者脑硫脂、胰岛素抵抗与颈动脉内膜-中层厚度的关系

摘    要:目的探讨2型糖尿病(T2DM)患者脑硫脂、胰岛素抵抗(IR)与颈动脉内膜-中层厚度(CIMT)的关系。方法对104例T2DM患者进行血糖、血脂、血尿酸(UA)、真胰岛素(TI)及脑硫脂检测,并行B超检查测量其CI—MT。结果CIMT正常58例,CIMT增厚46例。与CIMT正常者比较,CIMT增厚者的脑硫脂明显降低,年龄、血清TI及胰岛素抵抗指数(HOMA-IR)的自然对数In(TI)、In(HOMA—IR)]、C反应蛋白明显升高(P均〈0.05);多元逐步回归分析显示,CIMT与脑硫脂呈负相关,与年龄、In(TI)、In(HOMA—IR)、UA呈正相关。结论脑硫脂与IR有密切相关性,脑硫脂降低、IR是T2DM患者C/MT增厚的危险因素。

Abstract:Objective To investigate the relationship between serum sulfatide, insulin resistance and carotid intimamedia thicknesss of type 2 diabetes meUitus. Methods The fasting plasma glucose, blood fat, uric acid, true insulin and stdfatide in 104 type 2 diabetes mellitus patients were measured. The carotid intima-media thickness(CIMT) was measured by ultrasonography. Results There were 58 patients with normal CTMT and 46 patients with thickening CIMT. The sulfatid levels of patients with abnormal CIMT were lower than that of patients with normal CIMT. The age, serum true insulin, C reactive protein, In( true insulin) , In( insulin resistance index) and C reactive protein were higher in patients with normal CIMT than patients with normal patients( P 〈 0.05 ). Multiple regression analysis was displayed that CIMT was negativly correlated with serum sulfatide and positivly correlated with age, In( true insulin), In( insulin resistance index)and uric acid. Conclusions There is a close relationship between serum sulfatide and insulin resistance in patients with type 2 diabetes mellitus. The low level of serum sulfatide and insulin resistance may be the risk factor of CIMT incrassation of type 2 diabetes mellitus .
