Tp-e/QT比值在猝死高危患者中的预测价值

目的探讨Tp-e/QT比值在猝死高危患者中的预测价值。方法对30例猝死高危患者、30例健康成人（对照组）分别测量其入院24 h内的第1份心电图QTc、QTd、、Tp-e,并计算Tp-e/QT比值,分析其与恶性心律失常发生的关系。结果与健康组相比,猝死高危患者心电图上QTd、Tp-e、QTc、Tp-e/QT比值较对照组均明显延长。30例猝死高危患者中,发生恶性室性心律失常的有10例（33.3%）,心电图上QTd、Tp-e及QTc、Tp-e/QT值均较无恶性室性心律失常者有所延长,但只有Tp-e/QT比值显著增加。结论猝死高危患者中Tp-e/QT比值较健康成人明显延长,Tp-e/QT比值与恶性室性心律失常的发生有明显的相关性,可能是心脏性猝死的一个重要预警指标。

Tp-e/QT ratio as a predictive index of sudden cardiac death in patients

Objective To determine predictive value of the Tp-e/QT interval ratio in sudden death in patients at high risk. Methods Thirty healthy individuals and thirty patients with high-risk sudden death were recruited in this study.Their f irst QTc, QTd and Tp-e were measured within 24 h after admission to hospital,and Tp-e/QT ratio was calculated and analyzeal the relation between Tp-e/QT ratio and malignant ventricular arrhythmia.Results Compared with the healthy group,sudden death on the high-risk patients with ECG of QTd and Tp-e,of QTc,Tp-e/QT ratio compared with the control group were significantly prolonged.30 cases of sudden death in high-risk patients,the occurrence of malignant ventricular arrhythmias in 10 cases（33.3%）,the electrocardiogram of QTd and Tp-e and of QTc and Tp-e/QT values compared with those without malignant ventricular arrhythmias have extended,but only the Tp-e/QT ratio was significantly increased.Conclusion Sudden death of the Tp-e/QT ratio in the high-risk patients than in healthy adults was significantly prolonged Tp-e/QT ratio of malignant ventricular arrhythmias,the occurrence of significant correlation may be an important early warning indicators for sudden cardiac death.
