急性消化道大出血术中结肠镜检查的应用

目的探讨术中结肠镜在急性消化道大出血诊断治疗中的应用价值。方法对常规检查不能明确病因经内科综合治疗不能止血,需急诊剖腹探查的急性消化道大出血25例患者,在术中进行结肠镜检查,寻找出血原因和部位。结果 25例患者均成功地检出了出血部位和原因,其中小肠恶性肿瘤出血7例,小肠良性肿瘤出血5例,小肠溃疡、憩室等其他良性疾病10例,升结肠血管畸形1例,肝曲带蒂巨大息肉1例,溃疡性结肠炎1例。结论对不明原因的急性消化道大出血病人需急诊剖腹探查时,术中结肠镜的应用是一种操作简便,定性准确,病变检出率高的有效方法。

Application of colonoscopy during operation of patients with acute gastrointestinal hemorrhea

Objective To investigate the application value of intraoperative colonoscopy on diagnosis and treatment of acute digestive tract hemorrhage.Methods 25 patients with acute gastrointestinal hemorrhea were treated with intraoperative colonoscopy to look for bleeding causes and sites.Results 25 cases were successfully to identify the bleeding site and causes,including malignant tumors of the small intestine hemorrhage（7 cases）,enign tumors of the small intestine bleeding（5 cases）,small intestinal ulcer,diverticulum and other benign diseases（10 cases）,vascular malformation of colon（1 cases）,Hepatic flexure of the colon,huge pedunculated polyps（1 cases） and ulcerative colitis（1 case）.Conclusion The unexplained acute digestive tract hemorrhage patients requiring emergency laparotomy,intraoperative colonoscopy application is a simple operation,qualitative accurate,high effective method of detection lesions.
