中西医结合救治婴幼儿肺炎合并急性心力衰竭的疗效观察

目的:通过对比中西医结合救治婴儿肺炎合并心力衰竭的不同临床疗效。方法:随机将符合肺炎合并心力衰竭(心衰)的病例分为中西医结合治疗组和西医治疗对照组各40例,两组患儿均常规应用抗生素、强心、利尿、吸氧等治疗。治疗组在此基础上加用参附注射液1ml/kg静滴,2次/d,直到心衰纠正[3];并口服加味葶苈大枣泻肺汤,组方:葶苈子、大枣、车前子、炙麻黄、杏仁、地龙、丹参、红花、蚤休、鱼腥草。伴精神萎靡加朝鲜参;高热不退加蝉衣、生石膏;烦躁不安加龙骨、牡蛎;惊厥抽搐加羚羊角、钩藤;喉间痰鸣加僵蚕、仙鹤草;粪便干结加大黄。每日1剂,水煎2次,混合后分3～8次口服,连续服药3日。结果:治疗组在缓解呼吸困难、促进肺内微循环、消除肺水肿方面均较对照有显著性差异,并能加速心衰的纠正,缩短心衰时间,快速消除咳、痰、喘等临床症状。采用中西医结合治疗方法,在改善患儿缺氧状况、纠正心衰、消除肺部啰音等方面取得了较好的疗效,值得临床推广。
