超小剂量肝素预防儿童过敏性紫癜肾损害临床观察

【目的】观察超小剂量肝素预防儿童过敏性紫癜肾炎（HSPN）临床有效性和安全性。【方法】采用前瞻性随机对照方法，将97例过敏性紫癜（HSP）患儿分为超小剂量肝素治疗组（39例）。小剂量肝素治疗组（30例）和对照组（28例）。治疗10d。【结果】所有病例均随访9个月，超小剂量肝素治疗组发生肾炎4例（10．3％），小剂量肝素治疗组发生肾炎3例（10％），对照组发生肾炎9例（32．1％）。肝素治疗组间肾炎的发生率差异无显著意义（P〉0．05），但均明显低于对照组（P〈0．05）。超小剂量组所有病例均朱出现明显副作用。【结论】超小剂量肝素能预防或降低HSPN的发生。改善HSP的预后，无明显副作用。

Clinical Observation of Micro-dose Heparinum on Preventing Hypersensitive Purpura Nephropathy(HSPN) of Children

[Objective]To study the outcome of micro-dose heparinum on preventing HSPN of children.[Methods]Ninety seven cases were divided into micro-dose heparinum group(39 cases), small-dose heparinum group(30 cases)and controlled group(28 cases).They were treated for ten days.[Results]All cases were followed up for nine months. There were 4 glomerulonephritis in micro-dose heparinum group , 3 glomerulonephritis in small-dose heparinum group and 9 glomerulonephritis in controlled group , The two treatment groups had no significant difference(P<0.05) in occuring rate of glomerulonephritis,but were lower than controlled group significantly.And there was no side effect in micro-dose heparinum group.[Conclusion]The micro-dose heparinum can prevent HSPN effectively and safely.
