摘    要:目的 探讨食管鳞癌术后吻合口复发的因素.方法 收集我院2009年9月~ 2013年12月期间在我院行食管癌根治术后出现吻合口复发的患者,应用SPSS 19.0软件对复发情况与可能的影响因素进行κ方单因素分析和Logistic多元回归分析.结果 单因素分析显示,T分期、淋巴结状况(N)分期、切缘距病灶的距离以及肿瘤的分化程度等因素与食管鳞癌的复发有关;多因素研究发现,T分期及切缘距病灶的距离是影响食管癌复发的独立危险因素.结论 食管鳞癌术后患者出现吻合口复发的风险与术后T分期(T)、淋巴结状况(N)、病理分级(G)呈负相关,切缘距病灶越近,发生吻合口复发风险亦越高.

Analysis of high-risk factors for anastomotic recurrence in postoperative patients with esophageal squamous cell carcinoma

Abstract:Objective To investigate the risk factors of anastomotic recurrence in postoperative patients with esophageal squamous cell carcinoma. Methods Patients received radical resection of esoph- ageal carcinoma and had anastomotic recurrence were retrospectively reviewed in Yangzhou first people' s Hospital from Sep. 2009 to Dec. 2013. SPSS 19.0 software was used to analyze the recurrence and risk factors. Results Univariate analysis revealed a significant relationship between recurrence and 4 factors : T stage, lymph node status, distance between cutting edge and lesions, differentiation grade of tumor. Multi- variate analysis showed that T stage, distance between cutting edge and lesions were independent risk fac- tors for anastomotic recurrence in patients with oesophageal squamous cell cancer. Conclusion The anas- tomotic recurrence after operation was highly positively correlated with distance between cutting edge and lesions, but negatively correlated with T stage (T) ,lymph node status (N) and histological grade (G). The distance from cutting edge to the lesion is closer,the risk of anastomotic recurrence is higher.
