摘    要:目的 探讨组织多普勒技术评价冠心病患者腹主动脉(AAo)前壁运动速度及相对脉搏波传递速度的临床价值。方法 对95例冠心病患者的AAo前壁进行组织多普勒脉冲模式(PW-DTI)检查,结合同步心电图描记,分别测量PW—DTI频谱第一峰E、第二峰S1、第三峰S2和第四峰D的峰值;测量电-机械时间(EMT)、左心室射血前期(PEP)、脉搏波传递时间(PWT,PWT=EMT—PEP),根据坐高计算相对脉搏波传递速度(RPWV),并分别与健康组比较。结果 冠心病患者AAo前壁的PW—DTI频谱E、S1、S2、D峰值均下降,与正常对照组比较差异有统计学意义(P〈0.001);RPWV大于对照组(P〈0.001)。结论 AAo前壁DTI频谱参数及相对脉搏波传递速度可作为评价冠心病患者动脉壁弹性的新指标。

Abstract:Objective To evaluate the value of abdominal aorta wall motion velocities and relative pulsed wave propagation velocity(RPWV) in patients with coronary disease with ultrasound Doppler tissue imaging(DTI).Methods The velocities of abdominal aortic wall motion in ninety five patients with coronary disease were detected using Acuson Sequoia 512 ultrasound unit equipped with DTI capability.Conventional color Doppler imaging and DTI combined with electrocardiography were performed.Abdominal aortic wall motion velocities including systolic and diastolic velocity,electromechanical time(EMT),and pre-ejection period(PEP) were measured and averaged over 5 cardiac cycles.Blood pressure,sitting height,heart rate were measured and averaged over three times.Pulse wave propagation time(PWT) was equal to EMT minus PEP.RPWV was calculated,which was equal to sitting height divided by PWT.Results The characteristics of PW-DTI spectrum of abdominal aortic wall motion in patients with coronary artery disease were as follows: the peak velocity of E,S_1,S_2,and D were significantly lower than those of control groups(P<(0.001)).RPWV were significantly higher than those of control group.Conclusions The DTI parameters and RPWV of abdominal aortic wall may be new indices to assess artery wall elasticity in patients with coronary disease.
