实时三维超声心动图容积-时间曲线评价扩张型心肌病患者左心室收缩及舒张早期同步性

摘    要: 目的 应用实时三维超声心动图(RT-3DE)容积-时间曲线(VTC)评价扩张型心肌病(DCM)患者左心室收缩期及舒张早期同步性。 方法 对37例DCM患者(DCM组)和27名正常人(对照组)行RT-3DE检查,分析其VTC,得出左心室16节段、12节段、6节段心肌收缩期最小容积达峰时间标准差(Tmsv-SD)及舒张早期容积的时间标准差(Tedv-SD),用R-R间期标准化后,作为收缩期不同步化指数(SDI)、舒张早期不同步化指数(DDI)。 结果 ①与对照组比较,DCM组左心室收缩期及舒张早期不同步化指数差异均有统计学意义。②在DCM组中,心肌收缩期SDI及舒张早期DDI均与左心室射血分数(LVEF)呈负相关。 结论 DCM患者左心室既存在收缩不同步,也存在舒张早期不同步。
