全方向M型超声评价右心室流出道起源室性期前收缩患者左心室局部心肌功能

摘    要: 目的 应用全方向M型超声心动图评价右心室流出道(RVOT)起源室性期前收缩(PVB)患者左心室短轴局部心肌功能。方法 PVB组为RVOT起源PVB患者30例,对照组为30名健康志愿者。应用全方向M型超声系统测量两组窦性心律及PVB时左心室短轴乳头肌水平6个节段收缩期速度达峰时间(Ts)及舒张早期速度达峰时间(Td),评价PVB对局部心肌收缩与舒张功能的影响。结果 PVB组正常窦性搏动时与对照组比较,在下壁、后壁节段,PVB组的Td大于对照组(P<0.01),其他节段差异无统计学意义(P>0.05)。PVB组正常窦性搏动与PVB时比较,PVB时的Ts、Td均大于正常窦性搏动时(P<0.01)。结论 全方向M型超声心动图所测量的达峰时间能够反映RVOT起源PVB患者局部心肌收缩及舒张功能的异常。

Abstract:Objective To evaluate the regional short-axis myocardial function of left ventricular in patients with premature ventricular beats (PVB) from right ventricular outflow tract (RVOT) with omni-directional M-mode echocardiography. Methods Thirty patients with PVB from RVOT and 30 healthy subjects as control group were involved. The time to peak myocardial systolic velocity (Ts) and the time to peak myocardial early-diastolic velocity (Td) were obtained from the velocity-time curves of 6 segments of the left ventricular short axis view at the level of papillary muscles with OME, which were used to compared the changes of cardiac systolic and diastolic function of the two groups. Results Compared with that of the control group, Td of PVB group in the normal sinus beat was significantly longer (P<0.01) in the posterior and inferior segments, but no significant difference was found within the other segments (P>0.05). Compared with the normal sinus beat, Ts, Td were significantly longer in PVB (P<0.01). Conclusion The time to peak myocardial velocities can reflect the changes of regional systolic and diastolic function of the patients with PVB from RVOT.
