子宫内膜腺癌中PTEN和Bcl-2的表达及其意义

目的　探讨PTEN和Bcl 2在子宫内膜腺癌发生、发展中的作用。方法　应用免疫组化S P法检测PTEN和Bcl 2在 10例正常增生期子宫内膜 ,10例子宫内膜不典型增生 ,5 8例子宫内膜腺癌中的表达 ,并分析它们与子宫内膜癌临床病理的关系。结果　 (1)PTEN和Bcl 2在正常增生期子宫内膜、子宫内膜不典型增生和子宫内膜腺癌中的阳性表达逐渐降低 ,均有显著性差异 (P <0 0 5 ,P <0 0 0 5 )。 (2 )在子宫内膜癌中PTEN的表达与临床分期、组织分化、肌层浸润、淋巴结转移无关 (P >0 0 5 ) ;而Bcl 2均有显著性差异 (P <0 0 5 ,P <0 0 5 ,P <0 0 1,P <0 0 5 )。 (3)单因素分析PTEN和Bcl 2与子宫内膜腺癌患者预后没有明显的关系 ,多因素分析筛选出患者的年龄、手术分期是独立的预后影响因素。结论　PTEN和Bcl 2的检测对子宫内膜腺癌的早期诊断、恶性程度的判定和治疗提供了依据 ,与预后的关系尚有待进一步研究。

Expression and significance of PTEN and Bcl-2 in endometrial adenocarcinoma

Objective To investigate the influence of expression of PTEN and Bcl-2 on oncogenesis and progression in endometrial adenocarcinoma.Methods The expression levels of PTEN and Bcl-2 were determined using immunohistochemistry in 10 cases of nomal proliferative endometrium,10 cases of atypical hyperplasia endometrium and 58 cases of endometrial adenocarcinoma.Results (1)The expression of PTEN and Bcl-2 decreased gradually from normal proliferative endometrium to atypical hyperplasia,and adenocarcinoma,which showed significant difference(P<0.05,P<0.005).(2)The expression of PTEN in endometrial carcinoma showed no significant difference with clinical stage,histologic grade,myometrial invasions and lymph node metastases.But the Bcl-2 expression showed significant difference(P<0.05,P<0.05,P<0.01,P<0.05).(3)The univariate analysis revealed that the expression of PTEN and Bcl-2 was not significantly related to the prognosis of the patients.Cox proportional hazards model analysis showed that the prognosis was independently affected by age and surgical stage.Conclusion The experiment suggests that PTEN and Bcl-2 play roles in oncogenesis and progression of endometrial adenocarcinoma to different extents.Detection of these gene proteins has positive effects on early diagnosis,malignancy determination,and treatment of endometrial carcinoma.
