全麻机械通气中复张性肺水肿的临床探讨

摘    要:目的 探讨全麻机械通气中复张性肺水肿的发病机制、早期诊治及预防措施。方法 对4例复张性肺水肿患者进行回顾性分析。结果 4例患者均有单肺萎陷史,术中膨肺压力〉30cm H2Ol例,发生时间在肺复张后5-60min。所有病例均抢救成功,痊愈出院。结论 预防复张性肺水肿的重点在于术中通气管理,而充分的术前急救准备、早期诊治是抢救成功的关键。重视低氧时脑保护,对降低术后脑功能障碍有重要意义。

Abstract:Objective Research into the mechanism of reexpansion pulmonary edema in general anesthesia with mechanical ventilation to get earlier diagnosis and treatment. Methods Retrospective analysis of 4 cases with reexpansion pulmonary edema. Results All of them have the history of mono - pulmonary collapse. Pulmonary oncotic pressure were higher than 30cmH2O in l case. The edema happened from 5 to 60 min alter pulmonary reexpansion. And all of them survived and recovered. Conclusion The important of pevention of reexpansion pulmonary edema is the management of ventilation in operation. The key step of saving successfully is to have sufficient preoperational preparation and early diagnosis. Attaching importance to cetebro - protection during hypoxia has important significance to decrease dysencephalia postoperation.
