小腿静脉交通支功能不全的腔镜治疗

目的探讨电视腔镜下治疗小腿静脉交通支功能不全的方法及优越性。方法对18例下肢静脉曲张合并小腿静脉交通支功能不全患者先施行电视腔镜深筋膜下小腿静脉交通支离断术、然后再施行大隐静脉高位结扎与分段抽剥、小腿曲张浅静脉点状剥脱及经皮连续逢扎术。结果全组病例术后肢体症状和浅静脉曲张消失，6例足靴区静脉性溃疡在术后2周内愈合，切口无感染，全部病人足靴区色素沉着明显改善，平均随访16月无复发。结论电视腔镜治疗小腿静脉交通支功能不全手术具有安全简便、交通支离断彻底、损伤小、复发率低、并发症少等优点，有较好的临床应用价值。

Subfacifal endoscopic perforators surgery in treatment of lower limb perforating vein insufficiency

Objective:To evaluate the method and advantage of SEPS in treatment of patients with lower limb perforating vein insufficiency.Methods:Eighteen varicose patients with lower limb perforating vein insufficiency underwent surgery.Several surgical procedures were performed including SEPS?flush saphenofemoral ligation?limited stripping of the long saphenous and percutaneous continuous venule suture.Results:The surgery obviously improved venous disability, varicose vein and local pigmentation.Ulcers healed in 6 cases 2 weeks after surgery.All the cases have no surgical complications and no recurrence after 16 months following up averagely.Conclusions:SEPS is a safe and effective technique with advantages of minor injury,low complication rate and low recurrence rate.It dose have value in the clinic treatment of varicose with lower limb perforating vein insufficiency.
