| 摘 要： | 通过我院临床药师在参与消化内科临床医疗活动中,对一例肝硬化患者开展药学监护活动的实践经验,并结合药学服务理论知识,分析、总结肝硬化患者在药学监护中应关注的药学信息要点,为药师参与临床合理用药及开展药学监护工作提供参考和积累经验。

| Abstract: | The paper introduced a clinical pharmacist’s pharmaceutical care experience from a hepatic cirrhosis patient during his participating in clinical medicine activities of digestive system department. The paper analyzed and summarized the main points of pharmaceutical care information concerned in the pharmaceutical care by combining the medical practice with theoretical knowledge of pharmaceutical care. The experience and reference are thus provided for pharmacists who participate in clinical rational drug use and perform pharmaceutical care. |
