胆道疾病及脂代谢异常与2型糖尿病发病相关性分析

摘    要:目的探讨2型糖尿病(T2DM)患者与胆囊炎、胆结石疾病及脂代谢异常的相关性。方法2009年3月至2011年7月来我科住院治疗的280例患者分为T2DM组(140例)和非DM组(140例),对比分析两组胆囊炎、胆结石和脂肪肝的发病率及血脂水平差异。T2DM组按照其是否合并胆囊炎、胆结石及脂肪肝疾病分为甲组和乙组,对比分析两组血脂水平差异。结果T2DM组患者患胆囊炎、胆结石和脂肪肝三者至少1种的人数为97例(69.29%),显著高于非DM组的发病人数32例(22.86%);T2DM组的血清TG、LDL和ApoB的水平明显高于非DM组的研究对象;HDL和ApoA1的血清水平则低于非DM组的研究对象,且T2DM组中伴发胆囊炎、胆结石和脂肪肝的患者这种差异更明显。结论2型糖尿病患者中胆囊炎、胆结石和脂肪肝的发病率更高,发病原因与脂代谢异常有关。

Abstract:Objective To investigate the association of type 2 diabetes with cholecystitis, gallstone, and abnormal lipid metabolism. Methods 280 patients who had been treated during the period of March 2009 to July 2011 were divided into T2DM group and non-DM group. The incidence rates of cholecystitis, gallstones and fatty liver and the difference in blood lipid levels were compared between the two groups.The T2DM group in accordance with its merger of cholecystitis, gallstones and fatty liver disease is divided into Groups A and B, comparative analysis of differences in lipid levels in the two groups. Results The incidence rates of cholecystitis, gallstones and fatty liver were markedly higher in T2DM group (69.29%, 97 patients) than in non-DM group (22.86%, 32 patients). Serum levels of TG, LDL and ApoB were obviously higher and levels of HDL and ApoA1 were lower in T2DM group than in non-DM group, and the differece was more obvious in those with cholecystitis, gallstones or fatty liver in T2DM group. Conclusions Patients with type 2 diabetes have higher rates of cholecystitis, gallstones and fatty liver, which is correlated with abnormal lipid metabolism.
