MR扩散加权成像与CT增强扫描在肝硬化分级诊断中的应用意义

目的:分析肝硬化患者分别接受M R扩散加权成像、CT增强扫描诊在分级诊断中的价值。方法:本次研究针对目标选择为2015年7月—2018年7月间我院消化科收治的55例肝硬化患者,根据Child-Pugh分级标准将患者分为A、B、C三级,均分别接受MR扩扫加权成像检查及CT增强扫描检查,比较不同分级患者表现弥散系数差异及单动脉期、双动脉期CT扫描检出率。结果:肝硬化不同分级患者b=200s/mm^2、b=500s/mm^2表现弥散系数存在明显统计学意义(P<0.05),b=800s/mm^2时表现弥散系数差异无意义(P>0.05)。另外,双动脉期CT扫描检出率明显高于单动脉期检出率(P<0.05)。结论:MR扩散加权成像技术可用于肝硬化患者分级诊断中,CT增强扫描可提高病灶检出率,二者各具优势,但对人体辐射相对较大,临床需结合患者具体情况选取相应检查方法。

The significance of MR diffusion-weighted imaging and CT enhanced scan in grading diagnosis of liver cirrhosis

Objective To analyze the value of MR diffusion-weighted imaging and CT-enhanced scanning in patients with cirrhosis.Methods In this study,55 patients with cirrhosis admitted to our department of Gastroenterology from July 2015 to July 2018 were selected.The patients were divided into three levels according to Child-Pugh classification criteria:A,B,and C.The MR diffusion-weighted imaging and CT enhanced scan were performed to compare the difference in diffusion coefficient between different grades and the rate of CT scan in single arterial phase and double arterial phase.Results The diffusion coefficient of b=200s/mm^2 and b=500s/mm^2 in different grades of cirrhosis was statistically significant,(P<0.05).The difference in diffusion coefficient at b=800s/mm^2 was meaningless(P>0.05).In addition,the detection rate of CT scan in the double arterial phase was significantly higher than that in the single arterial phase(P<0.05).Conclusion The MR diffusionweighted imaging technique could be used for grading diagnosis of patients with cirrhosis.The CT-enhanced scanning could improve the detection rate of lesions.Both have their own advantages,but the radiation to human body was relatively large.The clinical examination needs to be combined with the specific conditions of patients.
