心脏直视手术后并发消化道出血的观察及防治体会

作者介绍2509例体外循环心脏直视手术后患者中9例发生消化道出血，占同期手术的0．35％，2例死亡。重点讨论了体外循环后消化道出血的发病因素为：术中体外循环时间过长，术后并发低心排综合征以及术前合并有胃溃疡和慢性胃炎等。治疗和处理以早期发现，及时观察和对症处理为主，重在预防。

The Observation and Management of Digestive Tract Hemorrhage in Patients after Open-heart Surgery

Digestive tract hemorrhage is one of fatal complications in patients with open-heart surgery.Patients with this complication have a high mortality rate of 54% to 63%. It is difficult to make early diagnosis and management for this complication. In this paper, 2 509 cases of patients who had undertaken open-heart surgery were reported, 9 cases of which complicated digestive tract hemorrhage.The incidence rate was 0. 35%, 2 cases died. The author discussed the factors of causing this complication and gave a special emphasis on early observation,early diagnosis and early treatment. The main causes were: a prolonged period of CPB (Cardiopulmonary Bypass), postoperative low cardiac output syndrome,and preoperative petic ulcer and chronic gastritis. The key measures of management were early observation,early diagnosis,early treatment and early prevention.
