桡动脉途径冠状动脉介入术后穿刺处包扎止血方法的循证护理

目的探讨经桡动脉途径冠状动脉介入术后穿刺口的最佳包扎止血方法。方法将92例经桡动脉冠状动脉介入术患者按手术时间顺序分为循证组（48例）和对照组（44例），循证组按照循证护理原则确定穿刺口包扎止血方法：用桡动脉止血带注气15州、定时减压、低压力水平维持8-12h包扎；对照组按传统方法包扎止血。观察两种包扎止血方法术后手部肿胀、麻木、发绀，穿刺口渗血、血肿，桡动脉闭塞，血管迷走反射等并发症发生情况。结果循证组手部肿胀、麻木、发绀和血管迷走反射发生率明显低于对照组，差异有统计学意义（P〈0．01或P〈0．05），而两组局部渗血、血肿和桡动脉闭塞等并发症发生率比较，差异无统计学意义（P〉0．05）。结论以循证原则确定桡动脉穿刺口的包扎方法，能减少患者血管并发症的发生率；以可靠的证据指导临床实践，达到提高临床护理质量的目的。
