摘    要:目的探讨抗骨质疏松治疗对糖尿病合并骨折术后患者的影响。方法将46例糖尿病合并骨折术后患者按治疗方法的不同分为2组:干预组和对照组,每组23例。2组均采用碳酸钙咀嚼片、骨化三醇胶丸、阿仑磷酸钠片和鲑鱼降钙素注射液治疗。在此基础上,干预组摄入富含维生素D、钙盐和适量蛋白质的均衡饮食。观察2组的骨折愈合时间、治疗前和治疗12个月后血钙、血磷、碱性磷酸酶、骨密度(BMD)及再骨折发生率情况。结果干预组、对照组骨折愈合时间分别为(15.2±3.6)周和(17.8±4.4)周,2组比较差异有统计学意义(P〈0.05)。2组治疗12个月后血钙、血磷、碱性磷酸酶水平比较差异无统计学意义(P〉0.05),干预组治疗12个月后BMD值与对照组比较差异有统计学意义(P〈0.05)。干预组再骨折发生率为8.7%,明显低于对照组的34.8%(P〈0.05)。结论对糖尿病合并骨折术后患者进行抗骨质疏松治疗,可改善患者骨密度值,缩短愈合时间,降低再骨折发生率。

Abstract:Objective To explore the influence of postoperative anti-osteoporosis therapy on diabetes patients with fractures. Methods Forty-six diabetes patients with fractures were divided into intervention group and control group, with 23 patients in each group. All patients were given chewable calcium carbonate tablets, calcitriol soft capsules, alendronate sodium tablets and salmon calcitonin injection. On this basis, patients in intervention group received a balanced diet containing vitamin D,calcium salt and appropriate amount of protein. The fracture healing time and incidence of refracture were recorded in both groups. In addition, serum calcium, phosphorus and alkaline phosphatase levels and bone mineral density(BMD) were determined before and after treatment for 12 months. Results The fracture healing time was(15.2±3.6) weeks in intervention group and(17.8±4.4) weeks in control group.The difference in fracture healing time was significant between the two groups(P〈0.05). No significant differences in serum calcium, phosphorus and alkaline phosphatase levels were found between the two groups after treatment for 12 months(P 0.05). However, there was a significant difference in BMD between the two groups after treatment for 12 months(P〉0.05). The incidence of refracture in intervention group was significantly lower than that in control group(8.7% vs 34.8%, P 0.05).Conclusion The poetoperative anti-osteoporosis therapy can improve BMD, shorten fracture healing time and reduce incidence of refracture in diabetes patients with fractures.
