NSCLC术后纵隔靶区调强适形放射治疗的优势

摘    要:目的通过对非小细胞肺癌(Non-small Cell Lung Cancer,NSCLC)术后纵隔内调强放射治疗(Intensity Modulated Radiotherapy,IMRT)或三维适形放射治疗(Three-dimensional Conformal Radiotherapy,3D-CRT)的肺剂量体积比较,指导临床选择合适的放疗技术。方法 2007年1月-2010年7月治疗的NSCLC术后纵隔行IMRT或3D-CRT放疗病人25例,所有病人均在危及器官(食管、心脏、脊髓)剂量体积相似情况下重新对应设计r3D-CRT或rIMRT计划(共50个计划)。采用剂量体积直方图评价IMRT与3D-CRT的全肺、对侧肺、同侧肺剂量体积和靶区适形指数(Comfort Index Value,CI值)。结果已完成3D-CRT与重新设计rIMRT计划比较,3D-CRT显著升高对侧肺V5(P=0.010)、V20(P=0.004),同侧肺V20(P=0.003)。已完成IMRT与重新设计r3D-CRT计划比较,IMRT全肺V10(P=0.019)、V20(P=0.002)、V30(P=0.002),平均肺剂量(Mean Lung Dose,MLD;P=0.003),对侧肺V20(P=0.038),同侧肺V20(P=0.000)、V30(P=0.002)等降低显著;IMRT具有更高CI值,提高了靶区适形度(P值分别为0.002,0.001);重新设计的rIMRT比r3D-CRT可应用计划更多(P=0.005)。结论 NSCLC术后纵隔内放疗时IMRT能降低全肺、对侧肺和同侧肺剂量体积,提高靶区适形度,从而减少放射性肺损伤发生的可能性,明显优于3D-CRT。
